Services

St. Michael Health provides Family Practice, Urgent Care, and Occupational Medicine using EKGs, echocardiography, and ultrasound. We also consult on surgical, orthopedic, and internal medicine. Additionally, we consult on highly aggressive cancer, Long Haul Covid-19 and mRNA shot injuries.

Ear pain?  We offer free ear checks for children ages 12 years and under.

Tier I- $135

 

  • Covid, simple cold, sore throat, pink eye/infection, seasonal allergies
  • Strep; test is additional $45.00
  • Influenza; test is additional $25.00
  • Urine infection; quick Dip Stick Analysis is additional $25.00
  • Simple strain or sprain
  • Established diagnosis of migraine or tension headache
  • Telehealth I
  • Rash/hives/acne
  • Impetigo
  • Ear pain/infection
  • Single ear irrigation/cleaning; supplies are additional $10
  • Medication refill
  • DOT physical exam
  • Well Child exam $100
  • High School sports physical $110

Tier II- $200

 

  • Post Op follow up
  • Fishhook or foreign body removals from eyes, nose, ears or skin; supplies are additional
  • Severe eye or skin infection
  • New patient medication refill
  • Medication consultation and/or a referral
  • Consultation or second opinion
  • Telehealth II (more than one issue or a referral)
  • Orthopedic injury not requiring labs or x-rays
  • Nose bleed; supplies are additional
  • Back pain not requiring imaging
  • Bronchitis
  • Double ear irrigation/cleaning
  • Carbon Monoxide Testing I

Tier III- $250

 

  • Adult and Pre-Op Physical. Additional EKG, echocardiography, or ultrasound procedures are $100 each.
  • Carbon Monoxide Testing II
  • New headaches or numbness of extremities
  • Simple laceration; supplies are additional
  • Skin tag, wart, mole, lesion, or cyst removal; supplies are additional
  • Incision and drainage of abscesses; supplies are additional
  • More involved orthopedic injuries requiring x-rays and/or labs; some labs and all x-rays will be referred to a nearby lab or radiology department.  Splinting, casting or bracing material are additional.
  • Back Pain requiring imaging referral
  • In-grown toenail; supplies are additional
  • Pneumonia, RSV (test is additional $25.00), or Severe Bronchitis

Tier IV- $300

 

  • Complex or Numerous Issues: 4 or more
  • Complex Laceration; supplies are additional

Tier V- Extended Visit $330-$430
