Tier II- $150
- Fishhook removal
- Other, foreign body removals from eyes, nose, ears or skin
- Skin or eye infections
- In-grown toenail, cyst or skin tag removal. (Cost may vary if there is more than one procedure.)
- Consultation (allows for 1 hour, 30 minute visit)
- Telehealth (1 hour)
Tier III- $175
- Adult physical exams. Additional EKG & echocardiography procedures are $100 each.
- Pre-op physical
- New headaches, numbness of extremities, or pain radiating from the low back
- Simple lacerations
- Mole removal
- Abdominal pain, diarrhea, nausea, vomiting or fever
- More involved orthopedic injuries requiring x-rays and/or labs. Some labs and all x-rays will be referred to a nearby lab or radiology department. Splinting, casting or bracing material will be charged at reasonable cost.
Tier IV- $250
- Complex lacerations
- Worker’s compensation exam, including two follow-up visits; supplies may be charged, at cost
